Abstract Title

Dr Sittana Elshafie is a graduate from the College of Medicine University of Khartoum, Sudan. She had an MSc in Medical Microbiology from the London school of Hygiene and Tropical Medicine (LSHTM), University of London, Fellowship of The Royal College of Pathologists (Medical Microbiology), UK, and a Post Graduate Diploma in Infection Control from LSHTM.She served as Clinical Microbiologis and Infection Control Consultant for more than 30 years. Dr Elshafie had training in Medical Microbiology at Hammersmith Hospital in London, The Royal London Hospital, and the Centers for Diseases Control and Prevention ,Atlanta Georgia USA. She also had training in Medical Education at the University of Maastricht, Holland , University of Gezira ,Sudan and Hamad Medical Corporation (HMC) Qatar. Dr Elshafie worked as Associate Prof of Microbiology at Weill Cornell Medicine -Qatar; Ass Prof of Microbiology at Sultan Qaboos University, Sultanate of Oman; Ass Prof Faculty of Medicine University of Gezira, Sudan; and a Senior Consultant in Microbiology and infection Control at: HMC; Aspetar ;The National Reference Laboratory, Qatar; Medani Teaching Hospital, Sudan; King Fahad Hospital, Saudia Arabia; Sultan Qaboos University Hopital, Oman; and Qimam Institute, Dubai. Administratively she served as Acting Laboratory Director, Head of Microbiology, Head of Infection Control Unit, Laboratory Safety Officer, Residency Program Director at HMC; and Laboratory Director at Aspetar Hospital leading the lab to be accredited by the College of American Pathologists (CAP). Dr Elshafie was a member of various committees National and International including Infection Prevention and Control, Food safety, Waste Management, Antibiotic Policy and Antimicrobial Stewardship, Pharmacy and Therapeutic Committee, Clinical Ethics, Research Ethics ,Laboratory Quality Improvement, Laboratory Standardization and Integration Committee. Internationally she is a member of the American Society for Microbiology;The Royal College of Pathologists , UK; Association for Professionals in Infection Control and Epidemiology (APIC) ,USA;The European Surgical Site Infections Society; WHO network of Problem - Based Community-Oriented Medical Education; and Member of the World Health Organization Advisory Group on Integrated Surveillance of Antimicrobial Resistance Advisory (AGISAR) Dr Elshafie had the opportunity to establish Diagnostic Microbiology service to Medani Teaching Hospital in Sudan and Sultan Qaboos University Hospital in Sultanate of Oman. She established the Infection Control Unit for HMC and The Infection Control Program for Aspetar . She chaired the Institution Review Board for the Ant-Doping Laboratory-Qatar and helped preparing the policies and guidelines for Research Ethics. Dr Elshafie main areas of interest are Antimicrobial Agents and Antimicrobial Stewardship, Laboratory Quality, Infection Prevention and Control, The Microbiome and Research Ethics. Reference: Bacillus cereus surgical site infections in orthopedics: Case report and Literature Review Surgical site infections (SSIs) are defined as infections occurring up to 30 days after surgery (or up to 90 days after implants surgery ) and affecting either the superficial or deep tissues and organs at the operation site. Despite improvements in infection prevention, SSIs remain a significant clinical problem in orthopedics as they are associated with substantial mortality, morbidity and can drain healthcare resources. The most isolated organisms in orthopedics are Staphylococcus aureus, coagulase- negative Staphylococci, and Gram-negative bacilli. SSIs caused by B.cereus in orthopedic surgery is rare. The organism can be introduced into the surgical site from the environment, medical equipment, or from the patient's own endogenous flora, if proper sterile techniques are not strictly followed. This may lead to serious complications like osteomyelitis or arthritis. B.cereus is an aerobic Gram-positive, spore-forming rod, widely distributed in the environment. It produces several toxins, including a necrotizing enterotoxin, an emetic toxin, phospholipases, proteases, and hemolysins which are important pathogenic factors. Although most known to cause gastrointestinal infections, it can also cause localized infections, and systemic infections. B.cereus produces beta lactamases and hence resistant to beta lactams including third generation cephalosporins. In this context surgical prophylaxis and management of infections with antibiotics other than beta lactams must be considered. While often dismissed as a contaminant, careful consideration is needed when B.cereus is identified in a surgical site culture, especially from specimens collected aseptically. This presentation describes a case of an athlete who presented with B.cereus SSI following surgery for closed displaced mid shaft fracture of tibia and fibula sustained during a football match, and a literature review. The presentation will also highlight the microbiology, the pathogenic potential, and antimicrobial susceptibility of B.cereus to increase awareness regarding the potential pathogenicity of this organism and the correct therapeutic and prophylactic approach.
