I graduated from the Medical University in Sofia, Bulgaria, in 1984 as the valedictorian of my class and received the Golden Hippocrates Statue and Medal. I initially worked as an Assistant Professor in the Department of Endocrinology at the same university, focusing on Internal Medicine and Endocrinology. In 1994, I moved to the USA and completed an Internal Medicine Residency at Lincoln Medical and Mental Health Center in the Bronx, New York. I then served as an Attending Physician and Chair of Medicine from 1997 to 2020 at John Peter Smith Hospital in Fort Worth, Texas. I taught hundreds of internal medicine residents, family practice residents, and medical students from both the hospital and the University of North Texas Health Science Center in Fort Worth. In 2020, I relocated to Las Vegas, Nevada. I am currently the Transitional Year Residency Program Director and a core faculty member of the Internal Medicine Residency Program at Mountain View Hospital in Las Vegas, as well as a member of the Sunrise Health GME Consortium. I am a board-certified endocrinologist and internist by the ABIM in the USA and the European Union. I have published 114 peer-reviewed articles on the pathogenesis and treatment of type 1 and type 2 diabetes, thyroid disorders, and male hypogonadism in the USA, France, England, India, and Bulgaria. My Continuous Glucose Monitoring Project received first place in the Quality category at the Far West Research Division Symposium in Las Vegas, Nevada, in 2021, 2022, 2023, and 2024. I have led three international multicenter scientific trials and authored six chapters on endocrinology in books published in England in 2021 and 2023. I am the Principal Investigator on a trial about the introduction of Continuous Glucose monitoring in an internal medicine Residency clinic, which is the first such trial in the USA. I delivered hundreds of lectures at various local, national, and international meetings on DM type 2, HTN, DVT, and related topics. I participated in 10 international scientific conferences on Diabetes, Technologies in Diabetes, and Primary Care in England, Austria, and Switzerland, delivering Webex presentations as a specially invited speaker. I worked in England with the renowned immuno-endocrinologist Prof. Franco Bottazzo, who discovered the Autoimmune nature of type 1 Diabetes Mellitus in 1974, from 1991 to 1992. In 1991, in his laboratory in London, England, I first observed increased expression of the adhesion molecule CD44 in snap-frozen sections and cell cultures from various autoimmune thyroid diseases. I am interested in a Program Director of Internal Medicine position with extensive teaching and clinical experience. I have served as an Associate and Full Professor for 28 years as an academic faculty member at hospitals and universities in Fort Worth, Texas; Dallas, Texas; and Las Vegas, Nevada. I served as Course Director in Endocrinology for second-year Medical Students at the University of North Texas Health Science Center (UNTHSC) in Fort Worth, TX, from 2017 to 2018, holding the rank of Professor. I have been a Professor in the Department of Medicine at the TCU and UNTHSC School of Medicine in Fort Worth, TX, since 2017. I am also an Adjunct Clinical Professor in the Department of Medicine at Touro Osteopathic Medical School in Henderson, Nevada, and a Full Professor of Medicine at the University of Las Vegas, Nevada. I was elected a Fellow of the American College of Physicians and recognized as a Top Doctor Internist for 2018 in Fort Worth, TX. Additionally, I was named Top Endocrinologist and Internist in Las Vegas, Nevada, in 2021, 2022, 2023, 2024, and 2025.
CGM Monitoring in Internal Medicine Residency Clinic: Prospective Trial - 4 Months of Data This prospective study aimed to evaluate how continuous glucose monitoring (CGM) use affects Glycemic Management Indicator (GMI) levels, which reflect HbA1c (estimated HbA1c), average blood glucose, and time in range (TIR) in patients who require multiple insulin injections—primarily those with Type 2 diabetes—and includes one patient with Type 1 diabetes mellitus. The main goal of the project was to assess the role that Internal Medicine Residents play in CGM management under an endocrinologist's supervision in a General Internal Medicine residency clinic. This approach has not been implemented in such residency clinics before and, if successful, could be expanded to other Internal Medicine residency programs across the USA and eventually integrated into the residency curriculum. Patients were recruited from the Internal Medicine Residency Clinic. Eligibility criteria included patients with an initial HbA1c level between 7% and 14% who self-monitor their blood glucose (SMBG) four times daily and qualify for use of a CGM device. Additionally, the study includes patients with Diabetes Mellitus who use three to four daily insulin injections, with or without other diabetic medications. After appropriate education by a board-certified endocrinologist, internal medicine residents remotely monitor patients who transition from SMBG to CGM. Patients share their CGM data with the Internal Medicine Clinic. Participants are contacted every two weeks by their designated resident physician via telephone and seen bi-monthly in the clinic by members of the CGM team. All residents work under the supervision of a board-certified endocrinologist, who oversees the project and is also part of the clinic. The study is planned to last 24 months. Currently, we have four months of data from 24 patients, providing insight into patient outcomes so far. The data show that after switching from SMBG to CGM at the four-month mark, the GMI decreased from 9.92% to 7.74%. The average blood glucose decreased from 275 mg/dl to 176 mg/dl, and the TIR increased from 30% to 57%. Introduction
