Public Health professional and licensed Pharmacist with integrated expertise in Reproductive Health (RH), Maternal and Child Health, and Medication Use & Safety. Holds a Master of Public Health (MPH) in Reproductive Health, with thesis successfully defended (November 2025). Experienced in evaluating medication use in reproductive-age populations, counseling on safe pharmacotherapy during pregnancy and lactation, and supporting community-based reproductive health and family planning initiatives. Strong interest in optimizing medication use to improve reproductive health outcomes and reduce preventable risks. In real-world clinical practice, pharmacists are ideally positioned to identify, prevent, and manage these issues through direct patient care and interprofessional collaboration. This practice-based presentation explores the role of pharmacist-led interventions in improving medication safety and public health outcomes. Drawing on hands-on experience in both hospital and community pharmacy settings, the session highlights common challenges such as inappropriate long-term medication use, drug–drug interactions, antimicrobial misuse, and gaps in patients’ understanding of their therapies. Key interventions discussed include medication therapy management, pharmaceutical counselling, deprescribing of high-risk or unnecessary medications, and support for antimicrobial stewardship programs. The presentation also emphasizes the pharmacist’s role in early risk identification, patient education, and collaboration with other healthcare professionals to optimize therapeutic outcomes. By sharing real-world experiences and evidence-informed strategies, this presentation demonstrates how pharmacist-led practices can enhance medication safety, support public health objectives, and contribute to more sustainable healthcare systems. The session aims to provide practical insights that can be adapted across diverse healthcare settings to improve patient outcomes and overall public health impact.
