Dr. Yesh Sharma is an Associate Professor and Consultant Endodontist in the Department of Conservative Dentistry and Endodontics at Pacific Dental College and Hospital, Udaipur, Rajasthan, India. He is a distinguished academician, clinician, and researcher with a BDS from Sri Siddhartha Dental College, Tumkur, Karnataka, and an MDS in Conservative Dentistry and Endodontics from Rajasthan University of Health Sciences, along with ongoing doctoral research focused on material sciences and micro-endodontics. He is a Fellow of the International College of Dentists (FICD) and the Pierre Fauchard Academy (FPFA). With over five years of academic and clinical experience, he specializes in advanced endodontic procedures including micro-endodontics, regenerative endodontics, retreatment cases, bioceramic materials, and aesthetic restorations. He has authored over 80 national and international publications indexed in reputed journals and is recognized for his research contributions in ozone therapy, NiTi rotary systems, and bioceramic sealers. He is a world record holder for his prolific research publications as a young dentist and has received multiple national and international awards for academic excellence and innovation. He is also a patent holder for a precision mapping device designed for clinical applications. In addition, he is an accomplished speaker and trainer, known for his workshop “The Obturation Odyssey,” and serves as the State Chairman of Rajasthan for the All India Medical Student Union (AIMSU), actively contributing to academic leadership, student development, and professional advancement in dentistry.
The Anatomy of an Apocalypse: Navigating Endodontic Failures Background: The ultimate goal of endodontic therapy is the elimination of pulpal pathogens and the prevention of reinfection. Despite high clinical success rates, endodontic failures persist, frequently presenting as complex clinical dilemmas that mimic a "biological apocalypse" for both the clinician and the patient. Salvaging these cases requires a profound understanding of the precise factors that lead to treatment breakdown. This paper deconstructs the multi-factorial etiology of endodontic failures and outlines a strategic roadmap for predictable clinical retreatment. Objectives: To analyze the microbial, mechanical, and biological mechanisms driving endodontic failures, and to evaluate contemporary techniques, imaging modalities, and biomaterials required to successfully navigate and resolve these clinical challenges. Discussion: The anatomy of an endodontic failure is categorized into three critical dimensions: Microbial Persistence: The survival of highly resistant intradental biofilms, predominantly driven by Enterococcus faecalis and Candida albicans, compounded by coronal leakage that re-contaminates the root canal system. Iatrogenic and Mechanical Deviations: The impact of missed anatomy (such as MB2 canals), uninstrumented spaces, ledging, zipping, perforations, and the mechanical obstruction of separated instruments. Obturation Inadequacies: The failure to achieve a three-dimensional, fluid-tight apical and coronal seal. Resolving these failures necessitates a paradigm shift from standard protocols toward advanced retreatment mechanics. This includes the utilization of Cone-Beam Computed Tomography (CBCT) for three-dimensional diagnostic precision, enhanced magnification, advanced rotary retreatment file systems, and aggressive chemical debridement using activated irrigation. Furthermore, the integration of bioactive materials, particularly bioceramic sealers and putties, is emphasized for their superior sealing ability, biocompatibility, and capacity to promote periapical healing in compromised roots. Conclusion: Endodontic failures are rarely random; they are the logical outcome of persistent infection or structural compromise. By systematically deconstructing the causes of failure and adopting advanced micro-endodontic retreatment strategies alongside modern bioceramic technologies, clinicians can confidently reverse the "apocalypse" and achieve long-term tooth retention.
