Left Diaphragmatic Eventration Mimicking a Giant Diaphragmatic Hernia: A Diagnostic Pitfall in a 52-Year-Old Male — Case Report   Diaphragmatic eventration is an uncommon condition in adults and may closely mimic diaphragmatic hernia on imaging. Accurate differentiation is essential because surgical management differs significantly. We report a 52-year-old male who presented with progressive exertional dyspnea and left-sided chest pain without prior trauma or thoracoabdominal surgery. Computed tomography suggested a giant left diaphragmatic hernia with apparent diaphragmatic discontinuity and intrathoracic displacement of abdominal viscera. However, intraoperative findings revealed an intact but markedly elevated and thinned hemidiaphragm, confirming diaphragmatic eventration. Diaphragmatic plication resulted in satisfactory lung re-expansion and marked symptomatic improvement. This case highlights the diagnostic challenge of distinguishing eventration from hernia in adults and emphasizes the importance of surgical correlation when imaging findings are equivocal.
